Every new program or grant received by nonprofit healthcare nonprofit entities results in additional needed infrastructure funding given that very few program-based grants cover ample administration and overhead. There is a basic philanthropic philosophy that a healthy nonprofit will include a solid mix of funding to avoid dependence in any one area. The HIV, Hepatitis, and STD communities have had a range of success at meeting this goal. At times because the states need the nonprofits to deliver the services needed by the population and mandated by the federal government, that a dependence creeps into the relationship forcing the nonprofit to give up independence in order to function more like an extension of the state. Health program design requires continued focus and work on independence for nonprofits – otherwise what happens as the funding structures change and the relationship with the states/funder change?   It is a 5-10 year challenge in some cases to turn this kind of dependence around, however it is critical that all program design include elements of independence from the state in order to stay connected to the community the nonprofits were established to serve. Sustainable program design can be done singly by a nonprofit or in concert with a state entity.
